I understand that in order to process my loan application MoneyShop is asking me to provide a personal information about myself and that MoneyShop will use that information to use Veda Advantage's credit reporting service to credit check me.

I understand that Veda Advantage will give MoneyShop information about me for the purpose of assessing my loadn application and that MoneyShop will give Veda information which will be held by Veda Advantage on their systems and used to provide future credit reports. This means that the information that I give MoneyShop and which MoneyShopy passes on to Veda Advantage will be given to other Veda Advantage customers who also use the credit reporting service.

I understand that MoneyShop may use Veda Advantage's credit reporting services in the future in relation to the provision of credit to me. This may include using monitoring services and receiving updates if any of the information held about me changes.

If I default in my payment obligations to MoneyShop, I understand that information about my default may be given to Veda Advantage and Veda Advantage may then give information about my default to other Veda Advantage customers.

I understand that I can access my personal credit information from Veda Advantage and that if it is incorrect, request that it be corrected. The information from Veda Advantage and that if it is incorrect, request that it be corrected. The information can be accessed through the website www.mycreditfile.co.nz or by writing to Veda Advantage at Private Bag 92156, Victoria Street, Auckland.

I understand that I am entitled a copy of any information that you may receive about me from third parties, and any information that you may give third parties about me.

I also authorise MoneyShop to contact the following people or organisations for the express purpose of processing my loan application:

• any other finance companies with which I have had previous dealings or appear to have had previous dealings;
• any government departments including but not limited to Social Welfare, LTS, Housing NZ, Immigration, Depart of Corrections and WINZ where it is directly material to my financial position;
• any government department including the Ministry of Social Development, for the purpose of taking lawful recovery action;
• any budget advisors I have used or use; my bank/s, my employer/s, the contacts I have listed and any other person or organisation about which I have provided information to MoneyShop in the process of making this application;
• any car dealer or any other agent with whom I am dealing in respect of the financing of property which is the subject of this application.

I understand the importance of providing the correct information to MoneyShop and that failure to do this will effect my load application and any loan contract I have with MoneyShop. I understand that MoneyShop retains a copy of all originating documents.
